CERTIFICATE (“Buy American”)

This form is used to document compliance with the “Buy American” requirement.

The instructions for submitting this form (if required) are included in 7 CFR 1726, Electric System Construction Policies and Procedures.

Fld Name / Item No.	Instruction
First Blank	Enter Borrower’s (Owner’s) RUS designation (e.g., AL001.)
Second Blank	Enter “Contractor,” “Subcontractor,” “Seller,” or “Material Supplier,” as the case may be.
Third Blank	Enter the number of contract.
Fourth and Fifth Blanks	Enter the date of the contract.
Sixth Blank	Enter name of RUS Borrower (Owner).
Signature Block	Enter name of the contractor, subcontractor, seller, or material supplier and the date prepared.  (Sign printed copy.)
