FARM STORED LOAN QUANTITY CERTIFICATION

This form is used to certify the quantity of farm-stored commodities pledged for a marketing assistance loan from FSA.

Submit the original of the completed form in hard copy or facsimile to the appropriate USDA servicing office.

Customers who have established electronic access credentials with USDA may electronically transmit this form to the USDA servicing office, provided that (1) the customer submitting the form is the only person required to sign the transaction, or (2) the customer has an approved Power of Attorney (Form FSA-211) on file with USDA to sign for other customers for the program and type of transaction represented by this form.

Features for transmitting the form electronically are available to those customers with access credentials only.  If you would like to establish online access credentials with USDA, follow the instructions provided at the USDA eForms web site.

Producers must complete Items 2 through 6, Items 8 through 15, and Item 18.

Enter the name of the individual producer, entity, partnership, joint venture, trust, estate, or corporation that is requesting a loan on farm- stored commodities.

Enter the address where the commodity is stored.

Enter the crop year in which the commodity was produced.

Type

If applicable, enter type of loan collateral.

·        Indicate if corn is cracked, rolled, or crimped.

·        HM = High Moisture ( applicable to high moisture barley, shelled corn and Grain sorghum loans)

·        ATHM = Acid treated high moisture (applicable to acid treated high moisture shelled corn and grain sorghum loans)

Lienholders

Enter lien holders name and address if there are any liens on the commodity. If there are no liens against the commodity enter the word "none" and initial.

Farm Numbers Where Produced

Enter the farm number where the commodity was grown. Contact FSA if you don’t know the farm number.

Other Payees

If you wish another person to receive part of the payment for this loan, enter the name of the person, and amount to be paid, and the ID number for payment. The ID number will be either a social security number or a federal tax ID number.

Seal

Enter the seal numbers assigned by FSA for each storage structure containing loan collateral.

 

Producer-assigned bin numbers may be used instead of FSA-assigned seal numbers. These numbers must match the assigned bin numbers on the CCC-683 "Commodity Loan Seal".

Bin Identifica-tion

Enter description of structure and location.

Example: Round steel bin next to barn.

Class, Variety, or Type

Enter the class, variety, or type as applicable. (Ex: grain, hay, silage, etc.)

 

For sunflowers, enter "Oil" for varieties of oil sunflower seeds that will be crushed for oil purposes. Enter "other" for varieties of confectionery sunflower seed.

Basis for Producer’s Quantity Determina-tion

Enter basis for your quantity determination, such as measurements, scale tickets, etc.

 

FSA must be satisfied that the producer’s basis for quantity determined is acceptable FSA will not accept manufacturer’s rated capacity of the structure as a determination of certified quantity.

Quantity in Bin

Enter the certification of quantity in the structure, as determined from entries in Item 14.

Please read the Producer’s Certification statement and answer the question, then sign and date where indicated.
