SUPPLEMENT TO FORM CCC-22-1, APPLICATION FOR APPROVAL OF WAREHOUSE.

Warehouse operators use this form when applying for a Peanut Storage Agreement to store CCC interest grain.

Submit the original of the completed form in hard copy or facsimile to the Kansas City Commodity Office, Storage Contract Branch, STOP 8748, P.O. Box 419205, Kansas City, MO 64141-6205; telephone 816-926-6446 or FAX 816-926-1426.
Customers who have established electronic access credentials with KCCO may electronically transmit this form to KCCO. Features for transmitting the form electronically are available to those customers with access credentials only. If you would like to establish online access credentials with USDA, follow the instructions provided at the USDA eForms web site.

Warehouse Operators must complete Items 5 through 13.

Items 1 through 4 are for CCC use only.

Items 5-13

5 Legal Name of Entity	Enter the legal name as shown on the CCC-22-1.
6 A – B Section and Location	Item 6A Enter the warehouse section identifier applicable to this CCC-22-1A. Item 6B. Enter the street address, town, county, and state where the warehouse section is located. If not located in town, give road, direction, and approximate distance from town.
7 A - D Owner or Lessee	Item 7A. Check "Owner" if applicable warehouse section is owned or "Leasee of Section" if section is leased. Item 7B. If warehouse operator leases warehouse section, enter the lessor’s name. Item 7C. Enter the expiration date of lease if section is leased. Item 7D. Check "Yes" if warehouse operator has an option to renew lease for section or "No" if not.
8 Direct Access to Rail Siding	Check "Yes" if warehouse has direct access to rail siding or "No, truck only" if not.
9 A - B Rail Shipments	Item 9A. Enter the name of railroad(s) and initial(s) on which rail shipments are received and loaded out if direct access is available. Item 9B. If no direct access is available but rail shipment is offered, enter the location and name of railroad(s) and initial(s) where rail shipments are received and loaded out.
10 A - D Control of Warehouse Space	Item 10A. Check "Yes" if warehouse operator has complete control of applicable section or "No" if not. Item 10B. If 10A is "No," enter explanation. Item 10C. Check "Yes" if applicable section is ready to operate, or "No" if not. Item 10D. If 10C is "No," explain why not and when the section will be operational.
11 A - D Construc-tion Type	Item 11A. Check if upright warehouse section is concrete, metal, or wood. Item 11B. Check if flat warehouse section is concrete, metal, or wood. Item 11C. Check if bins or tanks are concrete, metal, or wood. Item 11D. Enter other type construction or any additional descriptive comments regarding warehouse section’s construction.
12 A - H Operating Equipment at Section	Item 12A. Check "Yes" if warehouse operator has receiving scales for weighing and indicate the type of scales or "No" if warehouse operator does not have receiving scales for weighing. Item 12B. Check "Yes" if warehouse operator has shipping scales for weighing and indicate the type of scales or "No" if warehouse operator does not have shipping scales for weighing. Item 12C. Enter where grain is weighed for receiving and or shipping if Items 12A and or 12B are "No." Item 12D. Check "Yes" if warehouse operator has aeration equipment, or "No" if not. Item 12E. Check "Yes" if warehouse operator has drying equipment at this warehouse section. Check "No" if not. Item 12F. Check "Yes" if warehouse operator has permanent operating equipment at this warehouse section or "No" if not. Item 12G. Check "Yes" if warehouse operator has portable operating equipment at this warehouse section or "No" if not. Item 12H. Enter any additional comments regarding equipment at this warehouse section.
13 A - C Certification and Signature	Item 13A. Enter the signature of an authorized individual, i.e., corporate officer, partner, or proprietor. Item 13B. Enter the title of the person signing for the warehouse operator. Item 13C. Enter the date the CCC-22-1A is signed.
