CERTIFICATE OF COMPLETION, CONTRACT CONSTRUCTION

This form is used for the closeout of RUS Forms 200, 257, 786, and 830.

The instructions for submitting this form (if required) are included in 7 CFR 1726, Electric System Construction Policies and Procedures.

Section 1 (Page 1)	First Blank:� Enter the number of the contract. Second and Third Blanks:� Enter the date of the original contract. Fourth Blank:� Enter name of RUS Borrower (Owner). Fifth Blank:� Enter Borrower�s (Owner�s) RUS designation (e.g., AL001). Sixth Blank:� Enter name of contractor. Seventh Blank:� Enter the date of completion of the contract.
Section 7	Enter the total cost of the contract.
First Signature Block	Enter the name of the Architect or Engineer, the date prepared, the date signed, and the title of the signer.� (Sign the printed copy.)
Section 1 (Page 2)	Enter the date of the original contract.
Second Signature Block	Enter the names of the Owner and Contractor, dates, and title of the contractor�s signer.� (Sign printed copy.)
