Grantees complete this form.  Rural Development (RD) uses this from to identify the terms and conditions to be fulfilled by the Grantee upon award of a grant under the Renewable Energy Systems and Energy Efficiency Improvements Grant Program of the Rural Business-Cooperative Service.

Submit the original of the completed form in hard copy to the appropriate RD State Office.

2/Grant No.	Enter the grant number.  This is the numeric identifier for the grant.
3/Grantee Name	Enter the legal name of the grantee.
4/Address	Enter the address of the grantee.
5/Project Cost	Enter the total estimated project cost.
6/Grant Amount	Enter the amount of the grant.
7/Percentage Amount	Enter amount as percent of the total estimated eligible project cost.
8/Other Sources	Enter amount of funds available from other sources.
9/Location	Enter the location of the project.
10/Project Description	Provide a project description from the application.  This Grant Agreement shall cover the project described.
11/Real Property	Provide a description of any real property covered in this Grant Agreement.
12/Equipment	Provide a description of any equipment covered in this Grant Agreement
