Request for Single Family Housing Loan Guarantee.�

The purpose of this form is for lenders to request a loan guarantee from RHS, and certifying to RHS facts known by the lender.

 

Features for transmitting the form electronically are available to those customers with access credentials only. If you would like to establish online access credentials with USDA, follow the instructions provided at the USDA eForms web site.

A lender requesting an RHS Single Family Housing loan guarantee should complete this form and submit it to the appropriate RHS State Office. Mail all completed forms and any attachments to the address printed on the form.

Enter the total amount requested.� This amount should equal the guarantee request and will be equal to the amount on the �Loan Terms� page when a lender utilizes the Guaranteed Underwriting System (GUS).
