Instructions For CCC-502 Continuation

CONTINUATION SHEET FOR LEASED OR OWNED LAND

This form is used as a continuation sheet when additional space is needed for CCC-502A, CCC-502B, CCC-502C, or CCC-502D.

Submit the original of the completed form in hard copy or facsimile to the appropriate USDA servicing office.

Customers who have established electronic access credentials with USDA may electronically transmit this form to the USDA servicing office, provided that (1) the customer submitting the form is the only person required to sign the transaction, or (2) the customer has an approved Power of Attorney (Form FSA-211) on file with USDA to sign for other customers for the program and type of transaction represented by this form.

Features for transmitting the form electronically are available to those customers with access credentials only. If you would like to establish online access credentials with USDA, follow the instructions provided at the USDA eForms web site.

Producers complete items 1 through 11. Provide the identified information on the CCC-502 Continuation for each farm number recorded in Item 1. Attach this continuation sheet to the applicable CCC-502A, CCC-502B, CCC-502C, CCC-502D.

Items 1-11

Fld Name / Item No.	Instruction
1 County	Enter the name of the county where you are filing your CCC-502, Farm Operating Plan. Enter the farm number of the farm that is owned or leased.
2 Program Year	Enter the program year for the payment being requested.
3 State	Enter the name of the State where you are filing your CCC-502, Farm Operating Plan.
4 Producer’s Name	Enter the name of the person or entity filing the CCC-502, Farm Operating Plan.
5 Farm No.	Enter the farm numbers for all land owned or leased by the producer filing the CCC-502.
6 Name of County and State	Enter the name of the county and the state where the farm is located.
7 Land Owned, Leased To, or Leased From	Enter a checkmark in the checkbox to show that the land is owned, leased to, or leased from.
8 Name of Person Whom Land is Leased to and or From	Enter the name of the person you leased the land from or that you leased land to.
9 Cropland Acres	Enter the total cropland acres you lease and or own on this farm.
10 Rental Rate	Enter either the dollar amount per acre you pay the landowner or the percentage of the crop you pay for leasing the land.
11 Leased and or Owned Land.	Enter a checkmark beside this farm number if you leased this farm last year or if you owned this farm last year.
