Term

Definition

Accepted

These are your forms that have been received, reviewed and accepted by a Service Center for processing.  This does not mean your forms/applications have been approved.
You can view these forms, but you will not be able to change them since they have been accepted by a Service Center.

Add Forms to this Package

You can add forms that you have previously saved to the package you are creating.

Adobe Reader

This is a free software package (Adobe Reader 5.05) that you must have on your computer in order to view forms.  You can download the free Adobe Reader software from Adobe Systems, Inc., by clicking on the link on the eForms Home Page.

Attachment

You can attach a document to the form/package that you are submitting to a Service Center. Attachments must reside on your computer's hard drive and must be in standard file formats such as, .doc, .xls, .pdf, .tif, .jpg, .gif. The total size of the attachments can be no larger than 2.5mb.
The following file formats will not be accepted: .com, .exe, .bat, .pif, .vbs, .shs, .reg, .vbe, .wsh, .wsf, .scr, .lnk, .jse, .jsf.

Browse Forms

Select this option if you would like to browse the listing of forms or to find a form.  You can then print the form and complete by hand or you can complete the form online then print.  You will not be able to save or submit a form with this option

Cancel

This will return you to the previous screen.

Change a form after it has been submitted

You must contact the Service Center where you submitted the form/package for instructions. In most circumstances they will return the form to you using eForms and you can change it and resubmit. In a few instances processing may have moved forward to a point where another method will be needed.

Comments

You can add comments to the form/package that you are submitting to a Service Center.

Confirm Submission

In order to complete a submission you must click "Yes" on the Confirm Submission screen.  You are confirming that you do actually want to submit these forms to USDA and that information is true and correct to the best of your knowledge.  See the Confirm Submission screen for the full text.

Confirmation Email Notification

An automatic email confirmation of your submission will be sent if you supplied an email address during the submission process.

Confirmation Number

This is a unique computer-assigned number that identifies your form/package that has been submitted to a Service Center.  Keep a record of this confirmation number for future reference.  Be sure to include your email address when prompted if you want an email confirmation of your submission.

Contact Us

This option will provide information on who to contact depending on the type of question you have.

Create a Package

This option will allow you to group forms together for a submission. You can create a package that contains forms that you have previously saved.

Date Accepted

This is the date the Service Center accepted your form/package.

Date Last Saved

This is the last date that you saved the form/package.

Delete

This will allow you to delete a saved form or a saved package.

Description

This is the description of the form or attached file.

eForms

eForms is the World Wide Web location of forms commonly used by customers of Farm Service Agency (FSA), Natural Resources Conservation Service (NRCS), and Rural Development (RD). eForms is intended to be your resource to find forms, and instructions for completing those forms, on the internet.  Many forms can be electronically submitted to USDA.

Electronic Access Credentials

This is your USDA eAuthentication Account with Level 2 Access (User ID and Password).  See the eForms Home Page for instructions on how to register.

Electronic Signature

Forms submitted using your USDA Level 2 User ID and password are electronically signed.  This electronic signature has the same validity as a manual signature.

Email Address

Enter your email address when prompted if you want to receive email communications concerning your submissions.

Find Forms

This is a feature that helps you find and save forms for your own use.

Form Number

This is the number used to identify a USDA form.

Form Package

If you have more than one form to complete and submit to a service center, you can put all the forms into a form package and submit everything at one time.

This is a feature that allows you to search for online forms using various criteria.

History

This is the history of the form/package.  This will list when the form/package was submitted, returned, accepted, and so on.

ID and password

This is your USDA eAuthentication Account with Level 2 Access (User ID and Password).  See the eForms Home Page for instructions on how to register.

Instructions

This option will display instructions for completing the form.

Last Updated

This is the last time the form/package was updated.

Modify

This option will allow you to modify a form or package.

My Forms

This is the area where all of your forms/packages can be accessed.  It is sometimes referred to as My Forms Account.

Name Form

This is the name you assign to a saved form.

Name Package

This is the name you assign to a saved package.

New Submissions

These are your forms you have submitted to a Service Center that have not yet been accepted or returned.  You can view these forms, but you will not be able to change them since they have already been submitted.

Online Access Credentials

This is your USDA eAuthentication Account with Level 2 Access (User ID and Password).  See the eForms Home Page for instructions on how to register.

Offices

This option will link you to the Office Locator (Office Information Profile), which contains contact information for offices serving your area.

Package

This is a group of forms and attachments submitted to a Service Center in one submission.

Package Contents

This lists all forms and attachments included in a package.

PDF forms

This is the file format used to display forms on this site.

Print

This is the feature that allows the printing of forms, instructions, and screens.

RD

Rural Development

Reason

This is the reason the form/package was returned to you.  The form will need to be modified and resubmitted to the Service Center.

Redirected

This indicates that the form/package you submitted to a Service Center has been redirected to another Service Center for processing.

Redirect Email Notification

An email notification will be sent to you (if you supplied an email address during the submission process) when a Service Center Redirects a form/package to another office for processing. The email will include contact information for the Service Center now processing your submission.

Register

You will need to obtain a USDA eAuthentication Account with Level 2 Access (User ID and password) in order to save and submit forms online.

Resubmit

You use this option to resubmit your form/package to the Service Center.

Return Email Notification

An email notification will be sent to you (if you supplied an email address during the submission process) when a Service Center Returns your form/package.

Returned

This is your form/package that has been returned from a Service Center for changes or corrections. You can view, modify, or delete the form/package.  After all necessary modifications are complete, you can resubmit them to the Service Center.

Save

This option will allow you to save the form/package to your My Forms account. Your account resides on a USDA secure web server. Saving a form/package does not mean it has been submitted.

Save as

This option will allow you to save the form/package with a different name.

Saved Forms

These are your forms that you have saved to your MyForms account. You can view, modify, or delete the forms.  When the form has been completed, you can submit it to a Service Center

Search

This option will allow you to search web sites of the three agencies.

Service Center

An office of the United States Department of Agriculture that provides direct service to customers of the Farm Service Agency, Natural Resources Conservation Service, and Rural Development.

Use the "Offices" button on the eForms Home Page to find a Service Center near you.

Service Center Agencies Online Services

The Services Center Agencies Online Services Home Page can be accessed at www.sc.egov.usda.gov.  A variety of electronic services are available from this page, including eForms.

Sign In

Select this option to sign on to eForms using your USDA eAuthentication Level 2 Access Account (User ID and password). For more information about a Level 2 Access Account, visit http://www.eauth.egov.usda.gov/eauthCreateAccount.html.

Sign Off

Select this option to sign off eForms.

Signature

Forms that you electronically submit to Service Centers are also electronically signed.  Refer to each form's instructions to see if the form can be electronically submitted.  This electronic signature has the same validity as a traditional manual signature.

This option provides a brief demonstration on how to use this web site.

Status

This is the status of the form/package.

Submit

This option allows you to submit a single form to a selected Service Center. In order to submit a form you must have a USDA eAuthentication Account with Level 2 Access (User ID and password). Forms submitted in this manner are electronically signed. This electronic signature has the same validity as a traditional manual signature.

Not all forms can be electronically signed and submitted. Refer to the instructions for each form.

Submit Package

This option allows you to submit a package to a selected Service Center.  In order to submit a package you must have a USDA Level 2 User ID and password.  Forms submitted in this manner are electronically signed.  This electronic signature has the same validity as a traditional manual signature.

Withdraw a form after it has been submitted

You must contact the Service Center where you submitted the form/package in order to withdraw or cancel your application.  In most circumstances they will return the form to you using eForms and you can delete it.  In a few instances processing may have moved forward to a point where another method will be needed.
