What is USDA’s eForms website?

eForms is the World Wide Web location of forms commonly used by customers of Farm Service Agency (FSA),
Natural Resources Conservation Service (NRCS) and Rural Development (RD). eForms is intended to be
your resource to find forms, and instructions for completing those forms, on the Internet. Many forms
can also be electronically submitted to USDA.

What forms can I access through eForms?

FSA, NRCS, and RD have made hundreds of their commonly used forms available on-line.
By searching eForms, you can find the forms identified by each agency and program.

Forms approved for electronic signature can be completed and submitted online.
When you submit the forms are automatically transmitted to the Service Center
and given to the appropriate agency staff.
Forms that are not approved for electronic signature may be completed online and
submitted to a servicing office for review, however, you may be requested to sign the form
at a Service Center office.

What if my User ID and password does not work?

Verify that you are using the correct User ID and password. Passwords are upper and
lower case sensitive - are you typing in the right case?
If you should forget your User ID or Password, contact the USDA eAuthentication Help Desk by
email at eAuthHelpDesk@usda.gov for assistance.

What if I need help with a form?

Contact your USDA Service Center. If you need to find a Service Center near you, click on Offices at the top of eForms web site.

What are the timelines if I apply online?

The timelines are the same as if you applied in person or by mail for any USDA program.

How do I know what I need to submit online?

You should contact the USDA Service Center to find out exactly which forms are necessary
for the business you are trying to conduct or you can click on Find Forms and then use the Search facility to
help you find forms associated with each program.

How do I correct a mistake on a submitted form?

Contact your Service Center for appropriate guidance based on the status of your submission.

Can I use the eForms Service if I do not have an email address?
|

A: |
You must have a valid email address to obtain a USDA eAuthentication Account with
Level 2 Access that is required to submit the forms electronically.
Once you have signed in to the eForms service, you are not required to enter your email address.
However, you will not receive email notifications concerning your eForms account and will
need to check your eForms account frequently to look for activity. Having an email address
will enhance the level of customer service we are able to provide to you. Several commercial vendors
offer free email accounts and they are easy to obtain.

While submitting a form, my computer froze/acted up. What should I do?

Sign back into the eForms Service and click on My Forms - New Submissions. If the
forms you were working on appear here with a confirmation number, they were submitted,
if not, resubmit the forms.

My Forms account shows a submission as "Accepted", what does this mean?

A: |
These are your forms that have been reviewed by a Service Center and were
accepted as complete forms. It does not mean that the Service Center has
approved or determined your eligibility for any request related to the form.

What should I do if I submitted an application and have not received a response?

Sign onto eForms and click on My Forms to view the status of your submissions.
If you do not find adequate information here, contact your Service Center.

The Service Center sent notification of a service that I did not request. What should I do?

Click on the Offices button at the top of the eForm web site. This will link you to the
Office Locator (Office Information Profile), which contains contact information for offices
serving your area.

What assurance do I have that my information is secure?
|

The eForms Service security is similar to that used by financial institutions. Currently it is
running under Secure Socket Layer (SSL) version 3 with 128-bit encryption. Persistent cookies
are not stored on your PC.

What should I do if I suspect an unauthorized person is using my eAuthentication User ID
and password?

You must have Adobe Reader Version 5.05. Click on the free Adobe Reader

Can I send a letter or other document with my form?

Yes, eForms has an option to allow you to attach documents to your forms during the submission process.
Attachments must reside on your computer's hard drive and be in standard formats such as .doc, .xls
and .pdf, .tif, etc. The size limitation for all attachments on each submission is 2.5 MB.
The following file formats will not be accepted: .com, .exe, .bat, .pif, vbs,
.shs, .reg, .vbe, .wsh, .wsf, .scr, .lnk, .jse, .jsf.
