WOOL AND MOHAIR LOAN QUANTITY CERTIFICATION AND WORKSHEET
This form will be used by producers of wool
and mohair to certify the type, quantity, and identification of production
pledged to secure a CCC marketing assistance loan from FSA.
Submit the original of the completed form
in hard copy or facsimile to the appropriate USDA servicing office after each
producer has signed and dated (items 17A through 17H) and a representative from
the storing facility has read the certification statement (Item 18) and has
signed (Items 18 through 20B), if applicable, and dated the CCC-633WM.
Customers who have established electronic
access credentials with USDA may electronically transmit this form to the USDA
servicing office, provided that (1) the customer submitting the form is
the only person required to sign the transaction, or (2) the customer has an
approved Power of Attorney (Form FSA-211) on file with USDA to sign for other
customers for the program and type of transaction represented by this form.
Features for transmitting the form
electronically are available to those customers with access credentials
only. If you would like to establish
online access credentials with USDA, follow the instructions provided at the
USDA eForms web site.

2 Name and Address of Producer |
Enter the name and mailing address of the individual producer, entity, partnership, joint venture, estate, or corporation that is requesting a loan on farm-stored wool or mohair. |

3 Storage Location |
Enter the name and address of the facility where the commodity is stored. If stored on a farm or with a commissioned agent, enter the address of the farm or storage facility. |

4 Crop Year |
Enter the crop year in which the wool or mohair was produced. |
|
5 Commodity |
Check the appropriate box to identify if the commodity produced was graded wool, non-graded wool, or mohair. |

7 Lienholders |
List all lienholders including names and addresses if there are any liens on the commodity identified in Item 5. If there are no liens enter "none" and initial. |

10 Year Shorn |
Enter the year the wool or mohair was shorn. |

11 Type |
Enter the micron and yield which can be obtained from the core test results, if "graded wool" only was selected in Item 5. |

12 No. Head Shorn |
Enter the number of head shorn. |
|
|
13 No. Bags |
Enter the number of bags. |
|
|
14 Total Quantity |
Enter the total quantity, in pounds, that are being marketed for loan. |

15 Grand Total |
Enter the total from both Column 14’s. |

16 Producer Certification Statement |
The certification statement must be read by each producer signing the CCC-633WM and the question must be answered. |

17A - 17H Required Signature(s) |
Each producer who has an interest in the commodity identified in Item 5 shall read, sign and date the CCC-633WM certifying that the information on the form is true, and the certification statement has been read and understood. Please contact the FSA county office before signing and dating this document if you have any questions or concerns. |

Item 18 shall be read by a commissioned
agent(s) of the storing facility identified in Item 3, if applicable. Items 19A through 20B shall be signed and
dated by a commissioned agent(s) of the storing facility in which the
commodity identified in Item 5 is stored. Items 21 through 23B are for FSA use only. |
