Instructions for RUS Form 399

Supplemental Agreement to Equipment Contract for Field Trial

This form is used as field trial agreement between a borrower and an equipment supplier.

The borrower and supplier must execute three copies of RUS Form 399, and insert one copy in each of the three copies of the executed contract and specifications, and mail them to the Chief, Area Engineering Branch.

Input field 2	Enter the legal company name of the Seller (equipment supplier).
Input field 3	Enter the legal company name of the Purchaser (RUS borrower).
Input field 4	Enter the date of the associated material or equipment contract, e.g., 01-01-2001.
Item 1(a)	Enter the duration of the field trial in months. The duration of the field trial is stipulated in RUS For 399b, Telecommunications Equipment Field Trial.
Item 2	Enter the description of the item under field trial and its cost, including installation, if only part of the total contract cost is for material or equipment under field trial.
Seller	Enter the legal company name of the Seller (equipment supplier).
By	The space provided is for the signature of the authorized representative of the Seller.
Title	Enter the title of the authorized representative of the Seller.
Purchaser	Enter the legal company name of the Purchaser (RUS borrower).
By	The space provided is for the signature of the authorized representative of the Purchaser.
Title	Enter the title of the authorized representative of the Purchaser.
