Sixth Blank	Enter total amount of payments to the manufacturer, material supplier, or subcontractor.
Seventh Blank	Enter name of contractor
Eighth Blank	Enter date prepared.
Ninth Blank	Enter name of manufacturer, material supplier, or subcontractor. (Sign printed copy.)
